Side effects are troublesome symptoms or feelings that you may not expect that show up when you are taking a medicine.

All suspected side effects should be reported, especially those that are:

  • Unexpected, regardless of their severity, i.e., not consistent with product information or labelling; or
  • Serious, whether expected or not, i.e., that requires being admitted to the hospital, lengthened hospital stay, causes a birth defect, disability, is life-threatening or results in death;

      The use of baclofen for muscle spasticity is associated with a poor prognosis. A retrospective analysis of 30,000 patients with severe spasticity (i.e., multiple sclerosis) treated with oral baclofen for a minimum of 2 years showed a median survival of 3.5 years. There was also an increase in mortality with a median survival of 5.8 years, compared to 2.8 years for the placebo group. The use of baclofen was associated with an increased risk of multiple system organ failure. Patients with severe spasticity treated with baclofen had a higher incidence of death than placebo patients. The use of baclofen in these patients had a significantly higher incidence of hospitalisation, including death, in the baclofen group compared to the placebo group. It is important to note that patients who received baclofen treatment for muscle spasticity should be monitored for signs of renal impairment and should be given close monitoring during treatment. In patients with moderate spasticity, baclofen is not recommended as first-line treatment for patients with muscle spasticity who have poor renal function, as there may be an increased risk of renal impairment. In this population, baclofen may be preferred in patients with severe spasticity who have poor renal function. It should be taken into account that patients with severe spasticity who are treated with baclofen may have an increased risk of developing acute renal failure, which may be life-threatening.

      Baclofen is widely used as an oral treatment for muscle spasticity, but it is associated with a risk of multiple system organ failure. The risk of multiple system organ failure in patients treated with baclofen has been shown to be increased with longer duration of treatment, but is generally higher in the elderly (50 years), in patients with renal impairment (creatinine clearance (CrCl) <30 mL/min), and in patients with diabetes mellitus (diabetes duration <12 years). The increased risk of multiple system organ failure may also be seen in patients receiving baclofen treatment, especially those with a CrCl <30 mL/min (see ).

      The use of baclofen has been associated with a risk of a range of adverse events including hypotension, bradycardia, myoclonus, muscle cramping, and headache. The increased risk of multiple system organ failure is also known to be increased with longer duration of treatment. A review of the literature suggests that in these patients, baclofen may be used for longer periods of time, however, the risk of multiple system organ failure is increased with longer treatment duration and higher doses of baclofen.

      The combination of baclofen and metoprolol, a centrally acting inhibitor of skeletal muscle spasm, has been shown to reduce the risk of multiple system organ failure in patients with multiple sclerosis. However, it has been shown to have a different effect on the skeletal muscle. Patients treated with baclofen who have a CrCl <30 mL/min (CrCl 30-40 mL/min) have a lower incidence of death than patients treated with metoprolol (CrCl 30-40 mL/min). This suggests that caution should be taken when prescribing baclofen, particularly in the first few months of treatment, particularly in patients with severe spasticity. Further research is needed to determine the clinical significance of this finding, as well as the relationship of baclofen and multiple system organ failure to other health conditions. If the use of baclofen is continued despite the risk of multiple system organ failure, monitoring and management of the patient should be started accordingly.
